I made an order recently using all fake details and they requested a PR.
When you file a PR,
what sort of info do they ask for and is there a officer watching you fill it in?

Personal info / Name, Address, Details about what happened. You might be able to fill one out online if your district allows that. No, officers do not watch you fill it out.
