Personal info / Name, Address, Details about what happened. You might be able to fill one out online if your district allows that. No, officers do not watch you fill it out.

thank you, do you think im good to go in there and do it? last time i filled in a police action fraud report like this the opps showed up at my house, hope they dont come searching to the fake addy i put down

That's a totally different story that certainly warrants police action.

Filing a PR for SEing purposes (without any suspicion raised) Is no cause for concern.
