The killer computer bug - Skullmeat - 09-09-2020 The Therac-25, a computer controlled radiation therapy machine from 1983, had a software bug that caused it to deliver fatal does of radiation to patients. The machine relied entirely on computer control, and had no hardware based safety interlocks.
