The killer computer bug #1
The Therac-25, a computer controlled radiation therapy machine from 1983, had a software bug that caused it to deliver fatal does of radiation to patients. The machine relied entirely on computer control, and had no hardware based safety interlocks.
The problems eventually led to the IEC 62304 standard.

I vaguely remember reading something about this In the late 80s, though, not to this effect.

It's not difficult to Implement safety measures.

Man screw IEC 62304, that standard and regulation made me go bankrupt and go to jail for seven years for violating it
