Try to form habits. Start small and keep to it and add on slowly. Once you start, it's much easier to keep going. Eventually you should get to a point where you don't have to think about it.

Its hard to get out of a rut but try to plan little goals. Start small and keep building. Nothing more satisfying than checking things off a list!

Might be a hard pill to swallow, but it's as simple as getting off of your lazy ass and just do something. We have it way too easy nowadays.
Thing that helped me actually following through and staying productive was by setting goals for the day.

Diet, routine, and restful sleep are the most important in my opinion.

I thought I had the same problem, but I got diagnosed with ADHD recently. laziness isnt necessarily unfounded, in many cases it is adhd (which i will admit is overdiagnosed)
