It is the Classic sheet where they ask for name, surname, address and to insert if package has not been received or partial, indicating the items.  Then together they asked me for the photo ID

It's not an affidavit, but rather an Internal document generated by Zalando that's used for the purpose of claims In general.

This Is Identified by "Dear Customer" In their email copy, meaning a name Is not specified, hence It's distributed to anyone who submits claims similar to yours.

Is that something of concern? Today I will proceed to fill it in and send the identity document.

The copy of the email read "Dear Customer". In the original email there was "Dear (My name)". (I put customer to cover it)
