It appears to occur if a payment method or address, which has previous been associated with a couple of SEs, is used again. However, sometimes I cannot track down the exact reason why the account may be flagged. Can someone shed some insight?

how long would you usually "age" the account for and how many small orders before you place the big order?

I dont know why you get it to often. But I never seem to encounter this problem unless I drop a 400-500$ item on a newly created amazon account.
I always use 1 account per 1 phone and do 5-6 transaction per week for 3 weeks. $ can vary. I just buy rubberbands and shit to bundle my cash.
after 3 weeks I would hit big and usually always works putting down the order. refund part is another story.
