• Under "Category", choose "Other"
• Under "Product Version", put your type of White Strips, such as "Sensitive Teeth"
• Leave UPC code and production code BLANK!

4. Under "Question", you leave your story.

With his method they will say that the white spots happen and to contact your dentist if the issue persists. They wont give a refund or new whitestrips

When I called, I said black spots. That certainly got the job done.
