I've attempted a eyewear EB from mytheresa. They are requesting me to fill in a form by hand and return them a copy via email. Seems a little bizzare. Should I just fill it in and sign it with a fake signature?

I didn't go through with the refund on these in the end. At the time I had lots of diffrent SEs going on at once. I was being abit greedy and obsessive with it so I decided not to pursue a cupple of them.

Too bad, with that form that was a not legal binding one it would have been an easy catch

Judging by what you've described above, I believe you did the right thing to abort the SEs.
