Oh I see, well I am not exactly sure if it is affidativ but I suppouse so, there ain't much on the form itself just that I need to print it out and sign it with signature and name, also so you are not mistaken the whole order was ordered on a different name so I would be signing it with the same name as my order name but that ain't my real name.

Because I am not sure if it's just me being uneducated and there's nothing to worry about since the form doesn't look that scary and the order was not expensive as well.
This is not an affidavit, you can go ahead and fill it up, it is necessary to process your claim.
