What if the police report doesn’t tell the same address as the other ones. For example when I dna’d AirPods a week ago, they asked for the address of the crime (which would me my house), my name and email. I could change these details except my name if I get it delivered to an amazon locker.

It's still under your name, which means the PR belongs to the same person, Irrespective of where the alleged crime took place.

My name is a common name, they don’t ask for any ID when filing the report so how would they know it’s specifically me

It doesn't change the fact that the name listed on the report Is "your family & given name ", regardless of Its commonality and whether or not ID Is requested.
