Amazon UK FTID - 'Incident Report'? 10-14-2021, 09:25 AM

Hi all, I need some advice please.
I'm in the process of a FTID on Amazon UK. I've waited 2 weeks and checked the Royal Mail tracking (it says delivered). I contacted Amazon UK and they've asked me to fill in an incident report so they can complete an investigation. They want the following info:
Incident Report
1. Order number.
2. Date Incident Report was completed. DD/MM/YYYY
3. Date return item was shipped. DD/MM/YYYY
4. How did you ship back your return?
For example: Did you take the package to a drop off location, or did you leave it outside your house for the carrier to collect.
5. Condition of the item being returned.
6. Have you experienced prior issues with returning items to Amazon?
Has anyone experienced this before? I haven't seen it and I've done 3 or 4 FTIDs on Amazon now, all successfully and all immediate refunds after contacting them after 14 days. I'm considering just genuinely returning the item to be safe. Any advice would be great.
Thanks
