So let's say I create another account, buy the replacement item ("wrong item") directly from amazon, with different CC and different address so they can't link the two accounts.

Then claim wrong item and send it back.

They will scan the barcode and see that it's the same item that was shipped to another account, right?

That's where It ends.

If all goes well, you'll receive a refund/replacement.

Since they check inventory and every single item is identifiable. Doesn't that mean that the method will fail everytime they check?

They do not check their Inventory on "every" return.

They don't have the manpower to deal with the volume of claims, let alone check and do a stock take of their Inventory.

So how do you wrong item without the SKU being taken into consideration, or is it better to go defective route?
