I've claimed that I did not receive the item and today I received a form from Office asking me to fill out a disclaimer form looking something like this:

name:
Claimant (recipient)
order number:

I confirm that the good(s) ordered from (company name) on the (date) have not been

have not received the goods either.
I agree to co-operate with Yodel Security and/or the police in the event of any
investigation into the missing items.
I believe the above declaration is true to the best of my knowledge. I understand making a false
statement could lead to prosecution.

Should I sign it? Quite afraid of the 'prosecution' mentioned.
