When an account is banned does every address, every addressee (spelling contingent), every email, every phone number, every payment method, etc. ever associated with the account go with it?

Every detail will remain associated with the closed account.

In the event you create another account using any of those credentials and suspicion Is raised to warrant an Investigation Into Its history, Amazon will detect multi-accounting against the details (of the closed account) they have on record.
