I got an email saying they will process the claim but I will need to fill in a disclaimer form on a reciept of goods. 'I agree to co-operate with Yodel security and/or the Police in the event of any investigations into the missing items.'

Should I fill this out?

'I believe the above declaration is true to the best of my knowledge and belief. I understand
to make a false statement could lead to prosecution.'

Is It an affidavit, stat dec, denial of receipt form or otherwise?
