Do exactly that.
They're well aware of how to process the claim, hence they should've Informed you of their requirements right from the get-go.

Received a response today, asking for a haul away receipt from the company that disposed of the monitor to be able to pursue an alternative resolution. I don't recall any customer service requesting this, when they had just told me to dispose of the monitor. Proceeded to offer me the rebate alternative. What shall I do?

A ridiculous request Indeed.
At any rate, tell them that you've contacted the company and because the Item was disposed over a week ago, It's out of their hands, hence they're not able to provide a receipt.

They keep giving me the same response/run around. Im about to just come up with a bullshit story that I found the model tag sticker of the monitor somewhere because I feel like this rep. wont give at all.

You can request the claim be escalated, but first see how you go with the story you have In mind.

I requested, yet she claims that she is the highest department/personnel for escalation, which I thought was ridiculous. She tried to close my case , but I suggested I look for the monitor in my community's page (I told her I left it in the street)

If she's one of the reps who Initially handled your claim, then she's not part of the escalation team.
Claims are first assessed on a lower level and on request, they're escalated.

How did she respond to that?

She didn't reply, going to call their return department and see if I can go through with "finding" the tag and mailing it with the paperwork indicated. I don't know what kind of paperwork though.

Have a feeling they closed the case, the representative was really giving the run around and did not reply to my message pertaining what paperwork is needed.
