Hey everyone I was looking up info to help with a DNA I plan on doing with Walmart Canada and I came across a form someone had to sign regarding a missing order for Walmart Canada on Reddit.
Anyone come across this before? And what factors would need to be met to get hit with this document?

A flagged account would increase the likelihood of an affidavit. Value of the items, circumstances surrounding the DNA, and the age of the account as well.
