Amazon says that due to the value of "some items" they provide a OTP. Is it strictly a value limit? Or is it based on items most likely to be stolen?

It's based on your account history. I've found that items exceeding 600USD need OTP every time on most accounts

An OTP may also be required for Inexpensive Items due to either of the latter two circumstances above.

Nope, account history has nothing to do with it as far as I've managed to gather from various sources, they just put it up on random items (like, not all TV will have it, but the same TV will have it on ANY account)
It's a matter of price and risk, as @mothered said

It's a matter of price and risk, as @mothered said
I've know for a fact that the following happened:
Item 1 did not need an OTP, so the account holder DNA'd it.
After the SE succeeded, the account was unable to purchase it again without an OTP.
Might be that they added the OTP to the item just then.

That Is, they wanted to make sure the package was personally accepted with the second purchase.
