Asking for a request - Zurchi_DZ - 06-16-2013 These are just some guide lines I think should be followed when asking for a request.
Here's a sample request form if you want to use it. Code: Order Type: (Banner, Userbar, Signature, ect)

Sub-Text:
Colors Preferred:
Style Preferred: (Glossy, Dark, Metal, ect)
