Offspring threatening Prosecution? - Xx3003 - 10-23-2023

Require help on a DNA from Offspring/Office - totalling £160 (1 pair of shoes)
Courier: YODEL

I've claimed that I did not receive the item and today I received a form from Office asking me to fill out a disclaimer form looking something like this:

Claimant (recipient)
name:
Claimant (recipient)
order number:
Claimant (recipient)
delivery address:
Parcel tracking number:
Can be found in Your order has been
shipped email
Date dispatched:
Reference the date Your order
has been shipped email was sent
List of goods not received (SKU 15 digits) eg. XXXX
Item1:
Item2:
Item 3:
Item 4:
Item 5:

I confirm that the good(s) ordered from (company name) on the (date) have not been
received. I have checked with all the other members of my household and neighbors and confirm that they
have not received the goods either.
I agree to co-operate with Yodel Security and/or the police in the event of any
investigation into the missing items.
I believe the above declaration is true to the best of my knowledge. I understand making a false
statement could lead to prosecution.
Claimant (recipient) name…………………………………………..………………………………………………………

Should I sign it? Quite afraid of the 'prosecution' mentioned.

What will happen after signing this though?

You'll probably get your refund instantly or they'll investigate and give you a refund a few days later.
