In terms of the DNA, It's very common for Investigations to be opened.

It's simply part of the claims process, and so too Is the request to fill In a form. As long as It's not an affidavit, then you'll be fine.

Done a watch today too.
Bought it with an nike AF1, told them 10 minutes after receiving the package that I received the package but the watch is missing. Due to corona virus times the curier do not take my ID but asked for the number I told him my real one without problems. Zalando support asked me to sign their typical PDF documment that is a legal declaration blablabla, I done this before with fresh and old accounts, always different address, different phone numbers and different CC. In this case this zalando acc got 2500€ in bought items after the last SE that was done about a year ago. Hope this help you out to make a nice SE plan for the next time.
