Should I fill out that form? cuz I used my real name and info for the purchase and signature is need for the form I am kinda shally about the filling out the form, or should I just tell them I got the package and when I open it it is empty, go for the EB method instead? Thanks for the advice!

Why you think that signing is a problem? If you start feeling uncomfortable with the dna, in the worst case scenario you could say that a neighbor took the package and then give it back to the company.

I don't like signing anything, just base on my personal experience so I just wanna avoid that. And I think they don't offer free return so I prefer screening all the options before making that move

I will say it is an internal company investigation doc. The doc ask those general questions on refunder's refund form like order number, Dispute Explanation, tracking number, number etc, just a short one page doc. Only the signing part worries me tho currently

If It Is as you've stated, It's not a legally binding document, so there's no cause for concern.
