Windows 7 generally. Another will be an instance of linux, used for Asterisk VOIP PBX.

I don't know much about if ESXi/vmware works better for Windows, generally speaking Hyper-V is going to run it better since you have it tailored just for running Windows machines. The issue though is having to obtain a windows server license and keeping hyper-v licensed for what you're doing.

No no, primary OS will be debian. Id never run windows as a dedicated server. Only reason I need windows in a vm is for game servers.

Seems nice, although I always get 80+bronze/silver/gold/etc certified PSUs. I'd go with a cheaper case though, especially if aesthetics don't matter. As long as there's no obvious issues or really bad reviews, I don't see any problem in getting cheap cases, they're just boxes to hold your parts anyway.
