it's now been 2 weeks and i have not filled out the form cause i was scared , should i fill it out as it looks internal to footlocker (has the logo on the form on the top right ) and try to procede with my refund ?

if i do decided to fill out the form my excuse for being late is that the email did not send and i just checked since i did not get any updates ?
