As I was told by Customer Service and I’ve also read on here it’s only to move on with the claim, I’ll be signing it. If it continues to be a hassle I’ll take the L and move on. It seems this will be my last option. Delivery driver didn’t really see me nor did I sign for anything. Will check back in a bit.

Sorry for taking forever, affidavit was printed out and claim was accepted no more information was needed after sending the pic to their email. The carrier also seemed to know the the missing item claim was accepted because I asked them.
The items were for a laptop I tried to DNA at BB around $800ish
This DNA supposedly was successful but I still didn’t receive the refund, I was told that the claim was sent to the wrong department (I was ASSURED I’d get it after the 56th agent said they will send it to the right one) and to contact carrier. Ofc the carrier (Roadie) told me to contact BB so I ended up contacting my bank and filing a dispute. Dispute ended in my favor and still was able to keep the laptop.

Best possible outcome, well done.
